What is the Recovery Package?

The concept of the Recovery Package was developed because more people than ever before are living with and beyond a cancer diagnosis. The impact of cancer does not suddenly stop when the treatment is over. The Recovery Package is to assist people living with a diagnosis of cancer to prepare for the future, and identify their individual concerns and support needs. This would then enable people to return to as near a normal lifestyle as possible, including returning to work.

The assessment gives you the chance to think about your concerns and discuss possible solutions.
The assessment has three parts:

  • Answer a simple set of questions to see what concerns you have;


  • A discussion with a member of your healthcare team to talk about these issues and look at possible solutions. This leads to the development of a Care Plan;

  • The Care Plan will be agreed and documented by your healthcare team. You will receive a copy of this plan.

Treatment Summary

At the end of each course of treatment the patient’s hospital care team should create a Treatment Summary which includes alert signs for possible relapse, the effects of treatment, potential future issues, suggested actions for the GP and a key contact in the hospital for any questions or concerns. This is a key tool in patients self-management. The summary is sent to the GP as well as the patient so that the GP is aware of their role in providing continued care.

Cancer Care Review

This review is initiated by your GP or practice nurse and usually occurs about six months after your cancer diagnosis. The aim of the discussion is to check whether you have any concerns, need extra support or to provide details of local support services.

Health and Wellbeing

You may be invited to an event, where you and other people affected by cancer can find out more about healthy living and getting local support. The event can be a really good opportunity to meet other people in a similar situation, learn about local support and get useful contacts. They can take place in the hospital or in the community.

Hello. My Name is Lenny and I am the Transformation Project Manager. I am looking forward to working with local people as well as health and social care staff to improve outcomes for people living with and beyond cancer. My main focus will be to keep everyone on track over the next two years — making sure that we not only meet the required national standards but that the changes reflect what is important to people affected by cancer in Tameside and Glossop.

Hello. My name is Aoife and I am the User Involvement Coordinator. I have previously worked in Patient Advice and Liaison and Advocacy roles in both NHS and voluntary sector services. I am passionate about supporting people to ensure their views are heard. My main focus within this project is to facilitate active user involvement for people affected by cancer to ensure they are at the centre of service redesign and development.

Opportunities for Involvement 

 

The Project Team needs your help to improve the outcomes for patients living with and beyond cancer.  By sharing your experiences and opinions, you can help shape how the Recovery Package is implemented in Tameside & Glossop.

There are lots of ways to get involved and you can decide how much time you want to give. Your contribution could mean taking surveys, reviewing leaflets or acting as a patient representative at meetings. Either way, all of your actions will make a

big difference to the project’s work.

User Involvement Steering Group

User Involvement is a big part of the Recovery Package implementation work. We have therefore developed a User Involvement strategy and steering group.
